• Pending: UnitedHealthcare (currently out-of-network). A superbill may be available for possible reimbursement (plan dependent).

• Out-of-network (other plans): Discounted self-pay rate available. A superbill may be available for possible reimbursement (plan dependent).

• Not accepted: Medicare and Medicaid.

• HSA/FSA: May be used for copays, coinsurance, and self-pay nutrition services (including out-of-network visits).​
